The implementation of MACRA was the most notable change to Medicare physician payment in over a decade. Passed with bipartisan support, MACRA changed the way Medicare pays clinicians.

It established the Quality Payment Program (QPP) with two payment tracks—the Merit-Based Incentive Payment System (MIPS) and the Advanced Alternative Payment Model (APM) track. Payment adjustments started in 2019, based on performance measurement two years prior.
